for those who don't know and run into the same situation just check option 2A
Don't fill out all the information except the
DESCRIPTION OF MERCHANDISE IF MOTOR VEHICLE EQUIPMENT
And fill out all the information at the bottom of the form. Email or fax it out and you'll be good.
