I've got a 15mm bar and the brackets to attach it at the subframe, but where do the end links connect? Do I need to order bolts to connect it? None of the pics I've seen offer any real insight.

Make sure you also upgrade the front bar and keep the rear bar as small as you possibly can to reduced unwanted inside wheel spin and keep the rear suspension as independent as possible.
