Damage Notation Form
Not sure if this will print out okay, but I put this together just now and plan to use it any time I drop off a vehicle for any kind of service. Let me know what you think.

Definitely will be careful to note any damage on car before taking in for service next time. This has been a learning experience if nothing else!
