Unhappy Brand NEW Turbo Blown Off after 4000miles?

I have a 2008 BMW 135i FBO, JB4, JB4 MHD Backend flash on race map, alpina b3 flash, 5" vrsf fmic, vrsf catless downpipe, vrsf charge pipe and Tial bov, BMS occ, BMS intake.
The turbo system on the car had been replaced in a bmw dealership due to wastegate rattle. Turbos had about 4000miles on it before it blown off. This happened on a drag race, the rpm was about 6800. I used map 2 on the drag race this week, so it will make about only 17psi. The condition of my bmw is super good before this happened. Any thoughts what caused the turbo blown off?
I am also planning to replace or upgrade the turbos, any suggestions?

I wouldn't suspect the turbo blew from anything other than cooling issues.

Ive blown 3 sets of turbos in 10 months due to:

First set @60K miles
- Bearing failure, smoking.
Reason: Old turbos

Second set @67K miles
- Rear turbo bearing failed, tons of smoke out exhaust no boost.
Reason: Defective rear turbo

Third set @70k miles
- Rear turbo shaft play, loud siren sound and no boost.
Reason: oil line to rear kinked at the flex joint, nobody caught it.

Now on my 4th set i went with VTT stage 1 billet wheel, and im loving it. But i did replace front and rear oil lines to both turbos to prevent that issue altogether.

And im using Motul 0w-40 oil due to its high content of blah blah. Just good oil to cool the turbos and not get all gunked up in the feed/return lines. Happens to some n54's which blows the turbo from cooking it at high RPM.

Also i would shift before 6800rpm since its out of your high HP power band lol.

Sometimes it's a bad batch but other times it's usually the oil lines as another poster mentioned.

If I were to replace turbo's I'd definitely get new oil lines to the turbo's. It's money well spent as they are the life blood of the turbo's.
