I think it has to do with the angle under which he shot those. If I look at my Akras, they seem to be sticking way out when viewed perpendicular to the car, in fact adjusted them backwards but then they don't clear the diffuser when seen from above. The rear bumper/diffuser curves substantially, hence why it may seem like that from certain angles IMO.

No they are not adjustable but like someone said it's due to the angle of the picture. IRL they just stick out perfect. A bit more than the stock exhaust but not too far. The Akra of my friend his car do stick out more than this.
When the system is hot it also sticks out a half inch more.

And of course when stressed during driving, apart from heat expansion, there is also quite some exhaust movement:
