Blackline taillights

to anyone in the USA, schmiedmann has decided to ship via FEDEX and they are asking that I fill out this form for DOT: http://www.nhtsa.gov/cars/rules/import/hs799short.pdf

Any advice? Has anyone had to go through this?

I did not need to do a form in advance, but about 4 weeks after my order arrived, I got a note from FedEx that I owed them 15.00 for importing the box from Schmeidmann.
I forgot about it and after two more notices , sent them a check...

I also received a bill from fedex for $15, a couple weeks after I got the lights. Never had to fill out a form though.

thanks for the replies. Just want to know what to fill it out as so I don't end up having issues with how my car is classified now or in the future.

I got this form and sent it back via PDF, a few weeks later I got a $15 bill
