I believe the standard is still manual in Europe, too. When I was in UK in 2008, I never drove (not that I couldn't) and every car I rode with someone else was manual. Even the shuttle bus from the hotel to the airport was manual!!! And the bus driver was female. I think it's more like based on the generation and heavily on personal preferences. I am sure quite a few older women (let's say older than 40) can still drive manual, but I think they just absolutely hate it and refuse it.
