My dealer's policy is that you only get a loaner if you purchased the car from them. I'm almost always given an X3 or 328xi, although I got a 330i Sport once. All were automatics. Most had leatherette. None had iDrive.
I once got a Toyota Matrix from a local rental car company (dealership paid for it) because I showed up a little late and they ran out of loaners, but had promised me one.
