Loud Rattle on cold start while in gear

Hi my wife's 1 series 118i on start up when cold engine and in gear, a loud rattle emanates from the engine. When the engine warms up the noise completely dissappears. Also if car is left in park no noise is noticeable when engine is cold.

My local dealer seems to have been unable to locate problem even after changing all the belt tensioners.

Any one experienced this?

I have a 135i with N55 so obviously different engine, but I had something similar. I told it to the dealer when my car was in recently, and they did the following:
